Course Aims
Demonstrate a critical and systematic understanding of the case management of patients' health care needs across primary and community settings.
Course Content
This module will enable practitioners working in the community and primary care settings to develop the skills and knowledge required to undertake the case management of individuals and families with a range of health needs. Approaches to successful integration of multi-agency and multi-professional working as part of a case management approach to care will be examined.

This course can be studied at Level 6 or Level 7. In order to study at Level 6 you will need a diploma level qualification. In order to study at Level 7 you will need a degree level qualification. Students who commence the course have one month from the start date to transfer between levels, after this time transfer will be at the discretion of the Programme or course Lead.
